About the Role
As a Case Manager, you will be responsible for developing and implementing care plans in partnership with the consumer/representative across all of our Aged Care programs across the state. While drawing from your tertiary qualifications and experience, you will have a demonstrated understanding of current Consumer Directed Care principles, experience with working in the community Aged Care sector, as well as having strong communication and mediation skills and a current driver's licence.

Key Responsibilities

  • Undertake comprehensive consumer assessment and care planning within a psychosocial model.
  • Ensure appropriate information is given regarding the home care agreement and rights and responsibilities in acceptance of a home care package.
  • Represent Life Without Barriers and the Home Care Packages Program at external forums and meetings and promote the program within the community
  • Participate in regular individual supervision, performance reviews and team reflective practice sessions.
  • Support and promote the work of Life Without Barriers, maintaining a positive image of the organisation in accordance with level of position.
  • Monitor consumer budgets in relation to program guidelines.
  • Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.

Skills & Experience
  • Degree or equivalent in allied health, nursing, or a related field as well as previous complex case management experience
  • Demonstrated experience of working with frail elderly, people with culturally and linguistically diverse backgrounds and understanding of issues facing people with multiple and complex issues
  • Demonstrated knowledge, skills and experience in coordination and delivery of quality care in a community setting
  • Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for frail elderly, people with dementia and their carers, people with culturally and linguistically diverse backgrounds
  • Highly developed analytical skills and the ability to apply these skills in case management
  • Demonstrate alignment to LWB values
  • High level communication, influencing, liaison, negotiation and mediation skills
  • Current Driver's Licence

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Victorian Working With Children check.
